Good Chiropractic LLC is committed to respecting the confidentiality, privacy, and security of patient health information.

This policy describes the practice's general approach to patient information and is intended to supplement, not replace, rights and obligations established by applicable Colorado and federal law.

Our Practice Model

Good Chiropractic is a cash-pay chiropractic practice.

​

The practice accepts cash and payment cards and does not submit health insurance claims, conduct insurance eligibility checks, request insurance prior authorizations, submit claim-status inquiries, receive electronic insurance remittances, or otherwise communicate with health plans for insurance reimbursement.

Electronic Health Records

Good Chiropractic uses electronic practice-management and health-record technology to maintain clinical and administrative information.

We select systems and service providers intended to provide appropriate privacy and security safeguards and use reasonable administrative, technical, and physical measures to protect patient information.

Information We Maintain

Patient information may include:

• Patient identification and contact information
• Health histories, examination findings, clinical notes, care plans, and related records
• Appointment and communication records
• Consent and intake forms
• Payment and transaction records
• Other information reasonably necessary to provide and document chiropractic services

How Patient Information May Be Used

We may use patient information to:

• Provide and document chiropractic care
• Communicate with patients
• Schedule and manage appointments
• Coordinate care when appropriate
• Process cash or card payments
• Maintain business and accounting records
• Improve practice operations
• Meet professional recordkeeping obligations

We limit disclosure of patient information consistent with applicable law and professional obligations.

• With a patient's authorization
• With service providers as necessary to support practice operations and subject to appropriate safeguards
• With other healthcare professionals when legally permitted and appropriate for care
• When disclosure is required or permitted by applicable law

Patient Authorization

When applicable law requires patient authorization before information is disclosed, Good Chiropractic will seek appropriate authorization.

Patients may ask questions about what information is being requested or disclosed before signing an authorization.

Access to Records

Patients may request access to or copies of their records.

​

Good Chiropractic will respond in accordance with applicable Colorado law, professional recordkeeping requirements, and other laws governing the requested records.

Reasonable fees may apply when permitted by law.

​

Corrections and Questions

Patients who believe information in their record is inaccurate may contact the practice to discuss the concern.

Whether and how a record may be amended depends on applicable law, professional documentation standards, and the circumstances of the request.

Confidential Communications

Patients may tell us about reasonable communication preferences, such as a preferred telephone number or email address.

We will make reasonable efforts to respect those preferences consistent with safe and effective practice operations and applicable law.

Minors and Personal Representatives

For minor patients or adults represented by a legally authorized individual, access to and control of health information will be handled according to applicable law and the representative's legal authority.

Security

Good Chiropractic uses reasonable administrative, technical, and physical safeguards to protect patient information from inappropriate access, use, alteration, loss, or disclosure.

No electronic or physical information system can be guaranteed to be completely secure.

​

Electronic Communication

Email, text messaging, and other electronic communications may carry privacy risks.
 

Patients should avoid sending urgent or highly sensitive information through ordinary email or general website forms unless specifically instructed to use that method.
 

Electronic communications should never be used for medical emergencies.
 

Legal Requests and Required Disclosures

Good Chiropractic may disclose patient information when required or permitted by applicable law, including valid legal process, certain public-health or safety requirements, mandatory reporting obligations, or other legally authorized circumstances.
 

HIPAA Status

Good Chiropractic's current cash-pay model does not involve electronic health-plan transactions such as insurance claims, eligibility inquiries, prior authorizations, claim-status inquiries, or electronic insurance remittances.
 

Accordingly, this policy is not presented as a HIPAA Notice of Privacy Practices.
 

If the practice's operations change in a way that changes its obligations under federal health-information privacy law, Good Chiropractic will reassess and update its privacy practices and notices as appropriate.
 

Changes to This Policy

We may update this policy as our services, technology, business operations, or legal obligations change.

The current version will identify its effective date.
